FAMILY ID INSTRUCTIONS

Sports registration for all athletics and Band/Dance/Cheer/Choir is going on now. Students must be cleared through FamilyID in order to participate.
Please make sure you have completed all of the following:
 FamilyID Registration
 Updated Sports Physical
 Proof of Health Insurance
 Summer Camp Form (if participating)
Sports Physicals forms are available on the EHS website or at the front office and turned in to the Front Office.

The recommended method for registering on FamilyID is a laptop or computer. (If you use a phone, make sure you scroll down far and wide enough to click on "Save and Continue"). At the end of the process, you must click "SUBMIT" to complete the registration.

If your student played a sport last year, the information you typed in will auto-populate if you use the same email/account you registered the student with and the same "Student Profile."

Sports Physical: An original Sports Physical must be on file with the Athletic Clerk, Patricia Cardenas, at the front office. If you haven't already done so, please turn it in ASAP. A Physical is good for 365 days.

Proof of Health Insurance: This is mandatory. Through FamilyID, you can upload a front and back copy of the insurance card, or you can email it to vszabo@pylusd.org, or hand deliver a copy to the front office. If you don't have health insurance, you can sign up with Myers-Stevens, an affordable alternative at https://www.myers-stevens.com/
Driver's Information: On FamilyID, you must provide driver and auto information even if your athlete will be traveling via bus to away games. There are times when a bus may not be provided, like a weekend tournament.
